Hospice in the Weald supports people living with any illness that cannot be cured and may get worse over time. This includes cancer, heart or lung disease, neurological conditions, and other progressive illnesses that affect someone’s quality of life.
People can be referred to us at any stage of their illness. Hospice care isn’t only for the final weeks of life — many people benefit from our support much earlier, sometimes from the moment they are diagnosed.
Our care can include symptom management, emotional and psychological support, Living Well activities, complementary therapies, and support for family and loved ones.
At Hospice in the Weald, we know that the earlier patients are referred to us the better, and the more help and support we can give. We support people with life-limiting or life-threatening conditions at any point in their illness.
A referral could mean access to services within your community – helping you to live well, emotional support, and complementary therapies. Hospice care is not just inpatient care; our team can help someone from the moment they are diagnosed.

While a lot of referrals are through doctors, anyone can make a referral to the Hospice. Whether it is for someone you know or for yourself, do get in touch. Making a referral is easy, it can be done online or over the phone and will be handled by our Hospice Outreach Service.

We support people with any illness that cannot be cured and may get worse over time, not just those in the final stages of life.
Anyone living with an advanced or progressive illness – whether it’s cancer, heart failure, lung disease, neurological conditions or another condition that affects their quality of life, can be referred to our care.
If someone is finding things more difficult, whether that’s managing symptoms or coping emotionally, we can help. We also support family and loved ones every step of the way. This could be help with managing physical and psychological symptoms or emotional, practical, and spiritual support for them and their loved ones.
You can be referred at any stage of your illness, not just at the very end of life, and you don’t need a doctor or healthcare professional to make the referral; you always reach out directly to us.
Referrals can be made by a GP, by yourself or a loved one. To access all our services, the person being referred must be registered to a GP and live in our catchment area (covering Kent and East Sussex).
If you’re not certain whether someone is eligible or would benefit from our care, please still make the referral. Our team will review the information, confirm whether we can support the person, or help signpost to alternative services.
A person in need of inpatient care in the final weeks and days of their lives does not necessarily need to live or have a GP in our catchment area. If family or circumstances mean that Hospice in the Weald would be the best choice for a patient we can admit provided we have beds available.
We admit patients 7 days a week where possible. We can admit both weekdays and weekends as well as during the day and night depending on capacity and availability of beds.
