I need help to stay in my home

Our Hospice Outreach Service work closely with your GP, District Nurses and other Healthcare Professionals to support you & your family and help co-ordinate palliative care at home.

Our team is made up of a range of professionals and specialists who deliver care and advice most specific to your circumstances. We aim to minimise unnecessary hospital admissions by ensuring that plans, medications & equipment are in place before they might be needed based on individual need.

Staying at home is increasingly difficult

Our Cottage Hospital offers residential support and is described as either a home. Bring whatever they want and need to make their rooms feel the way they want. Whether that means bringing, photos, plants, or pets there aren’t many restrictions (as long as it’s safe and fits through the doors).

You can be as involved in the care giving as you wish but Nurses will always be on hand to help provide care and support to those staying with us, day, and night.

My needs are increasingly more complex

The In-Patient Ward at Pembury has 15 individual rooms for symptom control and end of life care. We offer personalised care to suit each patient’s physical, emotional and spiritual needs, as well as caring for and supporting those important to the patient. This might be family members, or friends.

Our In-Patient Ward is a calm, tranquil and peaceful place to be – almost a haven. Our rooms face out onto beautiful apple orchards, or into our stunning Hospice gardens and all have their own en-suite bathroom. We will always be flexible and accommodating for each family, whatever their needs.

I just need to talk to somebody

Hospice in the Weald recognises the enormous challenges of living with a terminal illness or life limiting condition.
The implications of managing treatments, side effects and coming to terms with changes for you or your loved one can be physically, emotionally, spiritually and socially exhausting.
And of course, we appreciate that it can be a difficult time for family and friends, as they adjust to changes around caring for loved ones or coming to terms with being without their loved one following their death. The Counselling and Support Service are committed to offering compassionate, personalised, holistic care to all patients and those important to them.

Is staying at home increasingly difficult?

When being at home is increasingly difficult we can provide residential care and support.

Your friends and family can be as involved as they wish in the care giving but nursing staff will always be on hand to help provide care and support to those staying with us, day, and night.

Patients have their own space and can bring whatever is needed to make their stay comfortable. This could be photos, plants, or pets there aren’t many restrictions.

Are your needs becoming more complex?

Our residential care team can provide full medical support for both symptom control and end-of-life care.

This is personalised to suit each patient’s physical, emotional and spiritual needs as well as caring for and supporting those important to the patient. This might be family members or friends.

This is a calm, tranquil and peaceful place to be – almost a haven accommodating each family, whatever their needs.

Hospice in the Weald recognises the enormous challenges terminal illness places on the patient and all those they care for.

When a loved one dies it can be difficult to know what to do. We will provide practical advice about what will need to be done. No one quite knows how a bereavement will affect them, but we are here to try and support you and to help you find a way forward.
