Thinking about the recent care you or a loved one received from the Hospice in the Weald, please would you complete this short series of questions. These questions are based on our Values and striving to deliver compassionate, personalised and holistic care. We will use this feedback to improve our Hospice Care, to inform what we do now and in the future. Any member of staff or one of our volunteers can help you to complete it if you wish. However, it is totally anonymous and we do not seek to identify the person completing the questionnaire.
