The Hospice Outreach Service Team
Hospice in the Weald is remaining up to date on the current outbreak of COVID and continues to follow advice from Public Heath England and the NHS. In some instances, we are taking additional precautions to safeguard our patients, families, carers and our workforce.
As part of these additional precautions, our Hospice Outreach Service are telephoning patients to provide support and advice over the telephone, and are only visiting patients and families who require essential visits providing there are no signs and/or symptoms of COVID within the household. Essential visits are agreed for patients requiring support in respect of pain, medications and/or end of life care. This is to ensure that we follow thorough infection control measures, and focus staffing and resources at our In-Patient Ward in Pembury to provide the best possible care to patients, families and carers.

The Hospice Outreach Service is a team made of Nursing Assistants, Staff Nurses, Hospice Paramedics, Clinical Nurse Specialists and Lead Clinical Nurse Specialists. The team have come from various backgrounds and specialities that include community nursing, oncology, heart failure, social care & pre-hospital care.
There is a wealth of experience and knowledge within the team that enable us to provide excellent care to support our patients and their families. The team are supported by the Hospice Medical team which is made up of Advanced Nurse Practitioners and our Doctors.
We endeavour to provide excellent palliative care to patients living within our notional catchment area and facilitate their wishes to remain at home with effective symptom management. We help create individualised and attainable care plans whilst keeping patients best interests at heart and to support family and friends through this time.
