Hospice Outreach Service

Support and advice in your own home, when you need it most.

Living with a life-limiting or terminal illness can feel overwhelming — not just for the person diagnosed, but for their loved ones and carers too. That’s where our Hospice Outreach Service comes in. We bring our expertise out into the community. Whether someone is living at home, in a care home, or staying with family, our Outreach team is there to offer support, guidance, and reassurance when it’s needed most.

Your GP remains your healthcare professional lead, we work alongside them to help you manage symptoms, make informed choices, and plan ahead with confidence based on your needs. Although we are available for support and advice we don’t operate emergency services.

What support is on offer?

Our team is made up of doctors, nurses, paramedics, social workers and other professionals, all working together to support people in the ways that matter most. We help manage symptoms, both physical and psychological and offer advice and guidance about palliative care, death and dying. We also support people with advance care planning, helping them understand their options and think about what matters to them, including where they may wish to be cared for in future.

We offer:

  • Emotional and practical support for both patients and those close to them
  • Access to a social worker to help with emotional and psychosocial needs
  • Support with benefits and financial matters, through our dedicated coordinator
  • Signposting to other services, both within Hospice in the Weald and across the community
  • Outpatient clinics, for those who can travel
  • Telephone support, when a home visit isn’t needed

Our aim is to help people feel informed, reassured, and more in control — whether they’re newly diagnosed, adjusting to changes, or approaching the end of life.

When is the support available?

Our service is available seven days a week, from 8am to 6pm.

While we are not a 24/7 or emergency service, you can still reach us out of hours at 01892 820 515 — Your call will be directed to the Inpatient Ward Team where a nurse may be able to help with your concerns. If there is an urgent concern or a delay in the call being answered, we advise you call 111. Any healthcare professional who might be with you can speak directly to the ward team for specialist advice.

Who is the service for?

We support adults living with any terminal or life-limiting illness, not just cancer. The service is also here for family members and carers who may need guidance or emotional support.

You might be referred to us at any stage — whether you’ve just been diagnosed, things are beginning to change, or you’re approaching the later stages of life.

How can someone access the service?

You can be referred by a healthcare professional such as your GP, hospital team, district nurse or other support worker. You can also contact us directly and self-refer, if you’re unsure what help is available — we’ll help guide you.

What to expect after the first contact

After receiving a referral, we will contact you to learn more about your situation. We will arrange a first assessment, which may be a home visit, a telephone appointment or a clinic appointment. Following this, we will plan with you the best ways in which we can continue to support you.

How do we work with your GP and other services?

We’re part of the wider healthcare system, and your GP remains the lead professional in your care. We don’t replace your GP, district nurse or hospital team, but work alongside them to offer additional specialist support.

We’re in regular contact with:

  • GPs and district nurses
  • Hospitals and specialist community teams
  • Out-of-hours and rapid response services
  • Local charities and support organisations

We don’t prescribe or dispense medication ourselves (except in urgent situations), but we can make recommendations to your GP. If other support would help, such as care at home, counselling or social care, we’ll help you navigate what’s available and connect you with the right people.

Our team includes professionals with specialist knowledge in palliative care — including Advanced Clinical Practitioners, Paramedics, Nurse Specialists, Assistant Practitioners, Social Workers, and a Benefits Support Coordinator — all supported by a clinical administration team who help coordinate care efficiently.

How long can people expect to be supported?

Patients can be referred at any time during their disease pathway when there is a need for specialist palliative care.

The team are here to support your specialist palliative needs. This might be over short periods of our advisory care, following which you are discharged until you next need us. Or it may be a longer period of support from the team. We treat everyone as an individual, focusing on what matters most to you.

You’re not alone

Facing a terminal illness can feel overwhelming, but you don’t have to navigate it alone. We’re here to help you stay at home if that’s your choice, support you and your loved ones emotionally and practically, and offer honest, compassionate guidance every step of the way.
