If you’re thinking of referring your loved one to Hospice in the Weald, please make sure you have their consent before you give us any information about them and their care. If they’re unable to give consent, the person who is appointed as their Health and Welfare Attorney under a Lasting Power of Attorney will need to make a referral on their behalf. If your loved one doesn’t have a Lasting Power of Attorney (LPA) for health and welfare, a healthcare professional will need to refer your loved one to us, following a Best Interest Meeting or Decision.

Once we’ve received your Access our Care submission, we’ll give you a call to gather a bit more information to see how we can support you. Please note at this stage we will only use the information you provide on this form to discuss how we can offer support.

We use the data you supply to:

Assess your referral and determine the services that may be right for your loved one.
Communicate with you about the referral and the care we can provide.
Coordinate with healthcare professionals, if needed, to ensure the best outcomes for your loved one’s care.

We take your privacy seriously and will only share your information with your consent or when legally required. Your data is stored securely, and we comply with applicable data protection regulations.
