Referrals to Hospice in the Weald
Hospice in the Weald is remaining up to date on the current outbreak of COVID and continues to follow advice from Public Heath England and the NHS. In some instances, we are taking additional precautions to safeguard our patients, families, carers and our workforce.
As part of these additional precautions, our Hospice in the Home team are telephoning patients to provide support and advice over the telephone, and are only visiting patients and families who require essential visits providing there are no signs and/or symptoms of COVID within the household. Essential visits are agreed for patients requiring support in respect of pain, medications and/or end of life care. This is to ensure that we follow thorough infection control measures, and focus staffing and resources at our In-Patient Ward in Pembury to provide the best possible care to patients, families and carers.

Patients are accepted to Hospice in the Weald if they have been diagnosed with a terminal illness such as:
- Advanced cancer
- Advanced degenerative neurological conditions
- Advanced heart failure
- End stage chronic kidney disease
- End-stage dementia
- Chronic obstructive pulmonary disease

Patients who do not have hospice palliative care needs but have generic end of life care needs are also accepted. For more information please see our Referral Policy.
Your GP, District Nurse, or other health care professionals may, with your consent, refer you to our Service. Patients and families can also make self-referrals (including for bereavement follow up), the Hospice in the Weald team would, with your consent, contact your GP to confirm medical history.

After you are referred to the Hospice, a team member will contact you by phone and arrange to see you at the Hospice in the Weald for an appointment. If you are unable to attend an appointment at the Hospice arrangements can be made to see you at home. After this first appointment, your care will be planned to meet your individual needs. You can speak to a nurse on the phone; be seen at a Hospice appointment or be seen in your home as frequently as you need. This can range from daily to once every few months.
