Most patients prefer to be at home if
they have the choice. The community nurse specialist
team provides advice and support to patients, families
and friends working very closely with the GP, Consultant
and district nurses at all stages. A specialist nurse
is always available on the phone during the night and
at weekends, because we believe that ready access to
a support team should remain at the heart of the service
we provide.
Our team consists of nine specialist nurses, all of
whom have extensive training in supportive care and
the control of symptoms. In addition the team includes
doctors, counsellors, and social workers.
The GP remains responsible for the patient care and
will continue to prescribe medication, while the District
Nurse/Home Carer will continue to provide support and
personal care.
Trained volunteers work with the home care team and
they are able to provide support whilst the carer has
an opportunity for respite.
