Day Therapy Centre
The Day Therapy Centre is a relaxed & safe environment for our patients and their families. We are a friendly and professional service aiming to improve the quality of life for those attending. Patients come for advice on symptom control, as a social environment to meet others in similar situations, for nursing care and to partake in creative activities. These may include pottery, metal work, jewellery and card making.
The service supports patients who are living at home. Patients normally attend one day per week between the hours of 10:30am & 3:30pm. There is full disabled access, a hoist and we provide a hot three course dinner. The department is open Monday to Friday from 8:30am to 5pm.
Patients are able to benefit from our support, enabling them to remain in their own homes as long as possible. We aim to improve their quality of life by mutual support and with input from our Community Nurse Specialists regarding symptom control. Carers are given a break knowing that the patient is in safe hands at the Hospice for the day.
The Day Therapy Centre patients are usually referred by the Hospice in the Home team. On the first visit all patients are assessed by the Clinical Nurse Specialist who is based in the Day Centre. Patients are assigned to a named nurse with whom care will be evaluated, planned and regularly reviewed. Assessment tools are used to determine patients health care needs.
The team in the Day Therapy Centre includes the nurses & volunteers, Palliative Care Doctors, Community Nurse Specialist, the Chaplain, Creative Therapist, Music Therapist, Health Care Assistants, the Occupational Therapist, the Physiotherapist, Complimentary Therapists, manicurist and a hairdresser. Together we aim to provide for all our patients needs whether they be physical, social, spiritual or psychological.
Patient are reguarly reviewed according to their individual needs. The Day Therapy Centre is not a long-stay unit and all patients are reviewed at the DTC multidisciplinary meetings every three months. Depending on individual circumstances some people then attend less frequently or are discharged from the Day Therapy Centre services. They can be re-referred to the Day Therapy Centre again in future and their palliative care needs will be re-assessed.
