Community Clinical Nurse Specialist Team
Most patients prefer to remain at home if they have a choice. With this in mind Hospice in the Weald has a team of 11 Clinical Nurse Specialist (CNS) who are based at the Hospice and work with patients, their carers and healthcare professionals. They visit patients in their own homes, at community and private hospitals, and care homes.
Our CNS's are a team of highly skilled nurses, who have undergone specialist training in palliative and supportive care and are very experienced in the management of symptoms that may be present as a consequence of suffering from a life limiting illness. They also provide emotional support to patients and their carers with the aim of improving the quality of life for patients.
As part of an advisory service each CNS works closely with and involves a patient's GP, District Nurse and Hospital Consultant. A GP will always remain responsible for their patient's care. Our aim is to complement this, not substitute it and the Team are available Monday - Friday, 9.00am - 5.00pm. After 5.00pm, at weekends and Bank Holidays, patients and families can telephone the Hospice for urgent advice, although staff are unable to visit at night.
