Caring for our Community

Patient Services

We aim to provide specialist palliative care to individuals with complex symptoms due to advanced progressive illness. This care extends beyond the individual to those important to them. Our care, whether as an In-Patient, attending our Day Therapy Centre or Out Patient Clinic, or whilst under the care of one of our Community Clinical Nurse Specialists or member of the Counselling Team, aims to incorporate the individual's physical, psychological, social, religious/spiritual and cultural needs.  Hospice in the Weald, therefore, provides a multi-professional service to those in need in our community.

We seek to:-

  • Have sensitive, open and clear paths of communication with patients, families and professional colleagues.
  • Deliver compassionate care of a constantly high standard.
  • Provide equal access to adult individuals with complex needs irrespective of gender, race, creed, religious or sexual orientation.
  • Care for individuals with unconditional positive regard.
  • Value and respect each person and to empower them and those that matter to them with information and choices.
