Practical Help Crisis Service
The Hospice also provides a Practical Help Crisis/Emergency service which is an integral element of our Hospice in the Home Service. Providing a more “hands on” approach to care when a situation deteriorates, preventing inappropriate admission to Hospital when this is not in line with the patient’s wishes.
The Practical Help – Crisis/Emergency Support Service is a new service set up by Hospice in the Weald. We are working with NHS West Kent to enable more patients to be cared for and to die where they choose and to reduce unwanted admissions to hospital. The service provides a 24 hour, 7 day a week service providing practical, domiciliary support in the home.
Our Hospice carer will assist or take the place of the informal carer in the event of a crisis. For example, they may act as a reassuring presence when a patient is entering the final hours of life, or they may stand in for a carer who has fallen ill.
The service is accessed via a qualified health care professional, who will, following a visit and assessment, contact our service.
Once the referral has been made, staff trained in triage will discuss the case with the referrer and run through a criteria checklist. Unfortunately we are unable to help with situations which require prescription/administration of medication or acute hospital intervention, the triage nurse will endeavour to direct the caller to the appropriate service. If we are able to help then we will aim to have a carer at your home within two hours of receiving the call and all relevant information.
The Practical Help – Crisis/Emergency Support Service provides extra support for a short period; this can be anything between a couple of hours to up to a week. The referrer and the Hospice carer will assess each situation and discuss the support available with you.
