Hospice in the Home

 

Hospice in the Home is an integral component of specialist adult palliative care, bringing the skills and supportive care associated with our Hospice into the home environment, thereby ensuring the patient and their carers’ are at the centre of the care. The service aims to enable adult patients with advanced life limiting illness to be fully supported at home, and to die at home if that is their preference. Nursing Care, advice and support may be provided to prevent admission to, or facilitate discharge from, inpatient care, for crisis management or for short periods of ongoing care. Patients and their families have the reassurance of 24 hour access to our skilled nurses and doctors who work closely with the patient’s GP and District Nurse.

A team of Clinical Nurse Specialists and Doctors work with the patient and their GP/Hospital Consultant providing expert advice and support in areas such as symptom management and the use of End of Life Care tools.
