If you would like to attend any of these courses and wish to apply for a place   please email eck-pct.eolc@nhs.net and request an application Form No. 1.  Alternatively complete the application form (Kent & Medway EoLC Training Programme form 1) and fax it to 01227 791299 or email it to eck-pct.eolc@nhs.net
