Hospice in the Weald provides compassionate, holistic, individualised and supportive care to anyone affected by advanced, progressive life-limiting illness. Once a patient is referred to Hospice in the Weald, they have access to the Community team, In-patient Unit, Day Therapy Centre, Counselling and Support Services, Lymphoedema Service and Complementary Therapies.
We use the Kent and Medway Network Specialist Palliative Care Referral Form which can be downloaded by clicking here.
Then the form can either be faxed to 01892 820520 or electronically completed and emailed to clinical@hospiceintheweald.org.uk
